You have the option of accepting our General Treatment Consent agreement form online during your client registration or you may Download, print, and complete our General Treatment Consent Agreement PDF form and bring it with you to your first appointment.

Surgery Consent

Download, print, and complete our Surgery Consent PDF form and bring it with you to the surgery appointment.
